Placement agreement form
The placement agreement form is the formal document stating that an agency has accepted you as a practicum student. In the semester prior to your practicum placement, you will need to complete and return this form to your field administrator.
Employment-based placements
If you are interested in completing a practicum in an agency where you are already employed, there are additional steps and information that need to be completed along with the placement agreement form.
Sonia
The University of Iowa School of Social Work uses a web-based data management system called Sonia, an online service for students, practicum instructors and placement agencies. This system is a way to maintain and share information about partnering agencies, supervisors, and students as well as track student learning activities from semester to semester.
This system helps students, practicum instructors, and agencies communicate and stay connected during the placement process. Because of this, it is important to become familiar with Sonia and keep all information entered about you or your agency current.
