Placement Agreement Form

The Placement Agreement Form is the formal document stating that an agency has accepted you as a practicum student. In the semester prior to your practicum placement, you will need to complete and return this form to your Field Administrator. 

Employment-Based Placements

If you are interested in completing a practicum in an agency where you are already employed, there are additional steps.

Intern Placement Tracking (IPT)

The University of Iowa School of Social Work uses a web-based data management system called Intern Placement Tracking (IPT), an online service for students, field instructors and placement agencies. This system is a way to maintain and share information about partnering agencies, supervisors, and students as well as track student learning activities from semester to semester.

This system helps students, field instructors, and agencies communicate and stay connected during the placement process. Because of this, it is important to become familiar with IPT and keep all information entered about you or your agency current.

Your default username and password will be provided to you by the School of Social Work via email. If you have questions about your username and password, please contact the practicum administrator in your center.

Privacy Notice
While IPT is a web-based practicum education software program, it is secure. Your personal information cannot be located through online search engines. When you first login to the program, you will use a default username and password. After signing in, however, you will be prompted to create a personal, confidential log-in code for added security. Your information in the software is viewed by the designated staff within the practicum education program only.

If you have any concerns about your privacy using the software or anything else regarding IPT, please contact the practicum education faculty at your center.

Timesheet Template

UISSW does not require students to track hour, but we strongly encourage doing so and some agencies require it. Maintaining a record of hours can be very beneficial if there are any disputes about how many hours a student has completed. The following template can be used to track your practicum hours.

Learning Plan

Early in your practicum you will develop a learning plan, which clearly lays out what you will be doing during the semester to meet the required competencies and competency indicators of the program. The actual document will be completed in the school's Intern Placement Tracking (IPT) Database system. Here are sample learning plans to help you develop your learning plan.

Site Visits

During your placement, you will be assigned a practicum liaison from UISSW who will conduct site visits with you and your practicum instructor to support your learning experience. The following forms represent what will be discussed during these site visits. These are for your reference only and will be provided at the site visits by your practicum liaison.
